*적응증 : 전자간 골절, 전자관통 골절, 불유합 및 부정유합 / 간단히 femur neck fx

*thread 없는 guide pin사용.

 

*처치

감마(아스로스타)

system + u-blade 1개

*준비물:

물품실 1: pkg2, 중방2, os minor or fx set, (F), #10, 4*8G, ioban, soap(3)

물품실 2: spoid(2), stapler, c-arm(head, body), D-gown(2)

그 외 : modular, 엘라텍스, vicryl 1, 2-0(317)

-중방 5개+ t-c 4 -> 소방 1 닦기 -> 중방 -> ioban

*procedure

1. guide pin + marker –check (ap)

ap check 하여 marking 한다.

2. guide pin + marker –check (lateral)

-C-arm 보면서 great trochanter에서 iliac crest 방향으로 2cm 그린다.

-lateral c-arm 볼 때 kelly 교수님 쪽 주기 / 반대쪽은 drap시 찝어 놓은 t-c

3. #10 mess skin incision

-사람의 BMI에 따라 incision크기가 다를 수 있다.

prn) kelly dissection

4. entry point에 guide pin을 mallet으로 넣는다.

entry point 보통 great throchanter 중앙보다 약간 위에있다.

5. PRN)entry point optimization (gudie pin c modular)

c-arm을 보면서 multi hole troca+ conical reamer sleeve로 위치를 4.0~5.5mm 조정할 수 있다.

6. preparation of the medullary canal

conical reamer를 사용하여 canal을 준비한다.

(pf 김 : manual X -> modular 사용)

nail preparation(보통 125사용, 175/180 및 길이는 그때그때)

-각도기 및 네일끼우고 6각(헥사)으로 뒤에 끼우기.

-적절하게 nail을 위치시킨다.

9. sheath를 주고 #10로 skin incision 후 gudie pin c modular로 drilling 한다.

10. deapth(흰색 플라스틱)를 주고 길이를 재고 drill을 조장한 다음 lateral cortex를 reaming한다.

*lag screw(U-blade type)

9. guide pin 뒤로줘서 guide pin 안 빠지게 하기

10. lag screw(u-blade screw) insert -> lag set screw c flexible driver -> u blade guide c T handle -> T handle 나오고 -> u-blade 날개 -> pusher -> T-handle로 빼기

-> u-blade 뒤에 꼬다리 flexible driver로 물어 준다.

-U blade screw 받으면 손으로 뺀다.

*prn) DKM 5.0 cannulated screw

-드라이버 와 guide 핀 작은 것 사용.

*distal screw

11. nail tip과 sleeve가 collinear인 것을 확인 한 후 disal sheath, mess #10 skin incision, 4.2 drill bit, depth, distal screw

12. end cap(보통 0) + 헥사 driver

13. irrigation + puspan

14. suture

vicryl 1(40mm), 2-0(26mm) stapler / 각 muscle. subcu, skin

-muscle은 regular tooth, subcu는 adson tooth

-Focus 및 의자주기(써큐)

dressing – wet G + dry G + 베타딘 볼+ elatax(5)
